Intensive Home Based Services Coach provides individualized, strength based interventions designed to improve mental health conditions that interfere with a child/ youth’s functioning and are aimed at helping the child/youth build skills necessary for successful functioning in the home, school and community.
Duties and responsibilities include but not limited to the following:
Perform direct service activities such as assessments, plan development, implementation, rehabilitation, etc.
Under the direction of the agency clinician and licensed clinician, utilizes behavior modification techniques to help the client achieve behavioral goals established by the clinician in the client care plan.
Intervenes with clients to enhance their ability to fully participate in treatment, including reinforcing treatment goals and conducting intervention strategies that support recovery, residency and placement stability in accordance with county/court plans.
Evaluate client responses to treatment, plans and modifies service activities as needed, and includes the client in the decision making.
Completes all IHBS progress notes as required by agency guidelines and ensures that all components required by the country mental/behavioral health dept. and state med cal are completed and submitted on time.
A bachelor’s degree in a behavioral science field is required. Ability to perform in-home services is required.
